Goals

1.Families served have their basic needs met and receive case management support. Our team conducts an intake session to create a customized Family Stability Plan, addressing the specific needs and barriers of each family. In 2025, we added a Vulnerability Scale to prioritize families who demonstrate the greatest needs, including length of homelessness, health, and children with special needs.
2.In 2025 and in 2026, FUSION is piloting a Peer Navigation Program to provide support to families after they exit our Family Center, to help them remain housed and to prevent a return to homelessness. Families choose to opt-in and receive counseling and support from a case manager with lived experience of homelessness.
