HEART2HEART'S VISION IS TO PROVIDE HOPE, SHARE LOVE, AND RESTORE PURPOSE TO THE AGING POPULATION BY MOBILIZING VOLUNTEERS TO C.A.R.E. FOR SENIORS.

Goals

1. 1.At H2H we believe that our social, emotional, model, of CARE is unique. We do not linearly focus on one area of care for the aging population but we strive to be a stop-gap service for any situation a senior is dealing with. Currently, we are serving 13k seniors a year and. they all have unique circumstances. For that reason, we are looking to expand and scale all of our existing programs.
2. 2.In 2023 we became the case management assessors for the Older American Meals home delivery program. Along with our food partners, we are seeing more than 500 seniors in their homes. As our seniors face more than food insecurity we want to be able to be a resource and advocate for all of their areas of need.
3. 3.Being a nonprofit that is not linearly focused on one area of seniors but looking at the seniors as a whole to make sure they do not fall through service gaps, can create a lot of things coming to us at one time. Something that we have learned and started to implement for our seniors and our partner agencies is to scale back completely on anything that is not core services and quickly refer out.
