Mission

Trenton Health Team (THT) is one of four state-designated Regional Health Hubs in the State of New Jersey, with a mission to improve the health and well-being of the greater Trenton community. Our vision for a healthy Trenton is rooted in the social determinants of health and includes: - Quality Healthcare - Equity and Social Cohesion - Healthy Environment and Housing - Vibrant Public Spaces - Growing Economy - Healthy Food - Improved Safety To realize our vision for a healthy Trenton, THT serves an integrator/backbone function, bringing together resident, healthcare, social service, and government partners to meet community needs.
