CARE GUIDE PARTNERS, INC. PROVIDED ADULT AND PEDIATRIC PALLIATIVE CARE IN THIRTY-FIVE COUNTIES IN KENTUCKY AND PROVIDED PEDIATRIC CARE IN TEN COUNTIES IN INDIANA.
