CONNECTED CARE OF SOUTHEASTERN MASSACHUSETTS WAS FORMED BY SOUTH SHORE HEALTH SYSTEM AND HARBOR MEDICAL TO OPERATE AS A NEXT GENERATION ACCOUNTABLE CARE ORGANIZATION (ACO) FOR MEDICARE BENEFICIARIES. THE ACO BRINGS TOGETHER THE HOSPITAL AND HEALTH CARE PROVIDERS TO PROVIDE HIGH QUALITY, COORDINATED CARE TO MEDICARE FEE-FOR-SERVICES PATIENTS.
