Mission

Our mission is to advance health equity by reducing cultural and language barriers to healthcare and resources in Cuyahoga County.

Our vision is for refugees, immigrants, and migrants to receive equitable healthcare in Cuyahoga County.

1.PGH will strengthen and expand CHW-led services that address social and structural barriers to care, guided by community voice and delivered through culturally and linguistically responsive practices.
2.PGH will increase equitable access to primary care, preventive services, and maternal–child health supports for individuals and families facing significant barriers to care.
3.PGH will reduce avoidable emergency department visits and preventable hospitalizations by strengthening care coordination, medication adherence, and chronic disease management.
