Mission

TO SUPPORT THE SOUTHERN HUMBOLDT COMMUNITY HEALTHCARE DISTRICT IN SUSTAINING HIGH QUALITY HEALTHCARE SERVICES IN OUR RURAL COMMUNITY.

Vision

SoHum Health Foundation is an all-volunteer community based 501c-3 nonprofit organization dedicated to providing financial and outreach support for the Southern Humboldt Community Healthcare District. Our Vision is three-fold: >To fund and support facility needs for optimal patient health. >To complete the Capital Campaign to build a new hospital and clinic. >To support healthcare initiatives and community grant-making in promoting health and wellness.

Goals

1.DEVELOP AND MAINTAIN A DIVERSE FUNDRAISING PROGRAM. 1) Build a multi-faceted annual fundraising program. 2) Complete Capital Campaign by following up on outstanding long-term pledges. 3) Maintain a donor program that includes donor portfolios, stewardship, and cultivation. 4) Host annual fundraising event. 5) Seek new donors, funding streams. 6) Create new fundraising initiatives as needs arise.
2.MAINTAIN ROBUST PRODUCTIVE RELATIONSHIP WITH HEALTHCARE DISTRICT AND SUPPORT THEIR VALUES, GOALS, AND PROJECTS TO PROMOTE OPTIMAL PATIENT AND COMMUNITY HEALTH. 1) Quarterly reports to District Governing Board. 2) Invite District employees to Foundation meetings, serve on committees. 3) Annual joint social with Governing Board. 4) Provide funding support for Healthcare District projects and needs.
3.CONTINUE TO BUILD AND STRENGHEN A SUSTAINABLE FOUNDATION 1) Create, implement, and maintain 3-year strategic plan. 2) Hold annual in-person board retreat to build community amongst ourselves and review priorities and goals. 3) Recruit new members who bring unique skill sets, backgrounds, and perspectives to the Foundation Board.
