UNDER AN AFFILIATION AGREEMENT DATED 3/29/2000 REVISED BY-LAWS DATED 2/14/2001 AND REVISEDARTICLES OF INCORPORATION DATED 12/2/2004, SUNNYSIDE IS NOW OPERATED,CONTROLLED AND SUPERVISED BY THE MARY LANNING HOSPITAL ASSOCIATION ANDIS OPERATED EXCLUSIVELY FOR THE CHARITABLE, SCIENTIFIC AND EDUCATIONALPURPOSES OF MARY LANNING MEMORIAL HOSPITAL WHO ALSO APPROVES ALL GRANTS.
